The work to be bid upon is described as follows:
• Construction of 840 LF of 6″ PVC Water Main, 3 Wet Tap Connections, 28 LF of bore with 10″ steel encasement, 1 – 6″ Gate Valves, 2 – 6″ Isolation Valves, a Fire Hydrant, 3 Dead Caps and other necessary items.
The Project generally involves replacing the ceilings, lighting, and HVAC grilles/diffusers to remove potential ligature points in selected unsupervised Patient areas; and renovations to the select Patient Restrooms and Shower Rooms. Replacing the aging horizontal hydronic piping mains associated with the heating and cooling system in the areas are part of add alternates.
This project is for the replacement of windows in room MN150 on the 1st floor of the Medical
Science Building, located at the University of Kentucky Medical Center Campus.
This project consists of the following:
a. Demolition of the existing masonry smokestack.
b. Demolition of the steel stair tower adjacent to the smokestack.
i. Infilling the existing steel railing around the existing concrete platform.
c. Demolition of the existing stair tower-mounted light pole.
i. Disconnect and terminate wiring inside the existing building.
ii. Remove exterior electrical conduit to light.
1. Patch holes, resulting from light and conduit demo, in existing brick with mortar.
d. Demolish existing steel wall plate.
i. Infill existing opening with brick featuring weep drains at base.
e. Infill existing basement wall opening with C.M.U.
f. Fill existing smokestack pit with flowable fill concrete.
g. Re-grade site to drain towards existing concrete drainage ditch.
i. Fill site with borrow soil as required to obtain drainage to existing ditch.
ii. Seed and straw site.
h. Cleaning the work area of all dirt and debris caused by the construction of this work is included in this contract. This work is to be performed through contract between the contractor and owner.

The project generally consists of:
Installation of an irrigation system throughout the Town Municipal Center. The water source for the irrigation system will be the two ponds located on the property. A pump station will be built on the south pond, to distribute the water throughout the entire irrigation system. The project will consist of installing the pump, intake line, concrete
pad and enclosures, controls and equipment, filter, floats, electric for pump and controller, boring, as sleeves as needed. The contractor will be responsible for the first winterization, spring turn on head adjustments, and controller maintenance for the first year. A maintenance class will also be given to the Towns maintenance department prior to final closeout.
The Project also includes site grading as indicated in the plans. There are 8 different
treatment options that range from adding 1- to 6-inches of soil. There are also two sites that require the removal of 4-inches of soil due to the amount of gravel incorporated into
the existing soil profile. The removed soil shall be disposed of according to the
specifications. The existing soil shall be reworked, which means to till and incorporate existing organic matter into the soil profile. The total soil needed for the site grading is approximately 1500 CY, and the soil will be supplied on site. Whatever soil is remaining, shall be spread around the area indicated in the plans. Once the irrigation and soil are put in place, all areas shall be seeded according to the specifications. Also, the removal of 47 ash trees to be replaced with 12 Black Tupelo (Nyssa Sylvatica), 11 Green Vase Zelkova (Zelkova Serrata), 12 Frontier Elm (Ulmus ‘Frontier’), and 12 London Planetree (Platanus x Acerifolia) trees.

This project will consist of replacement of the existing DOAS Units and upgrade of the existing Direct
Digital Controls System at the Perry County Justice Center. Generally, the replacement/upgrade consists
of work related to mechanical, testing and balancing, temperature controls and electrical.

The proposed project (approximately 11.0 acres total) consists of slide reclamation in Pike County. The project area is located on the United States Geological Survey (USGS) 7.5’ Belfry quadrangle (see the attached project and site maps).
The Brenda Phillips site consists of removing slide material, regrading, constructing a reinforced concrete wall, and ditch work around the Brenda Phillips residence. The site is located at 344 Pigeon Roost Road, near the community of Canada, KY. The project area is centrally located at N 37° 36’ 15.87”, W 82° 19’ 21.01”. To reach the site from the AML Prestonsburg office, travel South on US 23 toward Pikeville for 19.2 miles. Turn left onto US 119 toward South Williamson for 13.4 miles. Take the Sidney exit. Turn right onto Big Creek Rd. for 2.0 miles. Turn right onto Pigeon Roost Rd. for 0.4 miles. The Phillips residence is on the left.
Reclamation work consists of removing approximately 500 cubic yards of slide material, installing a 12’ tall, 70 L.F. long reinforced concrete wall. Subdrains and Concrete Block Tied-Mat drainage systems will also be constructed. Temporary access will be through the existing roadway stone driveway. The waste material generated during project work will be taken to a waste area off of US 119 (see the attached project and site maps).
During the process of these construction activities, trees over 5 inches in diameter at breast height will have to be removed in the AML problem area and to install drainage controls. A sediment and erosion control program, consisting of such measures as constructing hay bales/silt fence to be maintained throughout the life of the project and prompt re-vegetation using agricultural limestone, fertilizer, seed, and mulch will be implemented on all areas disturbed by this project.
Entry to the work areas will be accessed by the county road, residential driveways, and residentially maintained yards. The entire project work area slated for any excavation has been previously disturbed by any or all of the following: coal mining operations, including mine drainage, timber operations, gas/oil well development, residential development, road construction, and/or high velocity water flows associated with heavy rain events and flooding. These disturbances consist of significant upheaval, mixing,
and removal of earthen material from deep excavation, grading, sub-surface drilling, fill material placement, and erosion. There should be no undisturbed earthen material to a depth of several feet at the areas slated for significant project-related construction activities.
This project will include pre- and/or post- project maintenance and repair of existing previously constructed public county roads utilized during the construction of this project, which are administered by the county or local road authority. This construction activity will be conducted under the authority and supervision of the local public road authority and will consist of standard road construction practices appropriate to mitigate impacts to the local community from potential degraded road conditions in order to maintain public traffic safety.

building, which is already on site. Additional work to be bid for this job includes the excavation,

coordinate with Owner for MEP underslab rough-ins, which will be installed by Owner.

standby diesel generator, service entrance rated transfer switch, 20KW uninterruptible
power supply (UPS), and relocate an existing transformer together with the
appurtenances and related work at the Northern Kentucky water district’s Central Facility at 2835 Crescent Springs Road, Erlanger, Kenton County, Kentucky.

roof on the Louisville Metro Police Traffic Division facility.

The scope of the project includes the removal of steam and condensate piping in the portion of the utility tunnel from manhole #3 to the mechanical rooms of Kentucky Hall, Chandler Hall, Hill Student Center, and Hathaway Hall. In place of the steam and condensate piping, new chilled water supply and return mains and associated supports will be installed in the same location. Prior to substantial completion, the chilled water plant will be shut down and the new chilled water piping will be connected at the tunnel mains and within the mechanical rooms. Removal of tunnel lids and excavation of direct buried pipe will be required to support this scope. The tunnel lids will be closed by a separate contract. This project will require new direct buried pipe to be regraded to match existing grade and surface.

projects consist of milling, pavement patching, pavement placement, curb ramp replacement, curb,
sidewalk and driveway replacement, pavement striping, and other appurtenant work associated with
pavement improvements to various streets throughout the City.
The primary purpose of this project is to demolish and replace two existing electrical services each consisting of a primary switch, 300KVA 4160V/208V transformer and 1200A/120/208V/3P/4W distribution panel. Both services are located on the first floor concourse level of Freedom Hall.

This project consists of work required to rehabilitate the portions of OCEA’s gravity sewer collection system. The
work includes but not limited to sewer cleaning works, sewer CCTV taping and review, manhole rehabilitation,
point repairs, sewer lining, service lateral grouting and lining, and sewer replacements. Sewer range in size from 6
inches to 10 inches diameter.
Work shall include all scheduling, road detour planning and implementation, products, equipment, materials and
labor necessary to complete the work including all restoration work necessary to restore all property and roads to a
minimum of pre-existing conditions.
